As far as dumping sewage, garbage and untreated gray water, even though this may be allowed by law (which is apalling unless it's an emergency), this is almost never, if not never done by any reputable cruise line unless it's absolutely necessary (for example the ship is stuck out at sea for 2 extra days because of a hurricane in it's path, and the storage tanks are filled to the brim). I mean come on, what reputable cruise line is going to engage in these practices when their image is on the line in an already dismal travel economy? They would surely be caught and most likely boycotted by several environmentalist groups. This would have already happened if this was truly a widespread problem.
