Another way is to get a local (in the country you're visiting) prepaid SIM and a calling card such as the ones from clonecom or enjoyprepaid. Those cards have local phone numbers in most big cities, so you just call that number (charging a low local rate to your SIM) and connect to the US number through the calling card (usually something like 5 cents/min). I did this in Germany and Austria this past summer using an unlocked blackberry I got off ebay cheaply and it came out to about 15 cents/min total.
The other advantage of local prepaid SIMs is that you can call restaurants, confirm hotel reservations, etc.
