As a native New Orleanian and veteran of many carnivals, I always recommend to visitors to bring and wear hiking boots. Mardi Gras involves a LOT of walking and the more support and protection for your feet, the better. I concur on the idea of a light, lined parka - something to keep you dry and warm if the weather changes (which it always does). Add sunblock to the list, too. If you go into the Quarter, bring only your driver's license, a credit card, your hotel room key, and cash, put them in your front pockets, and, ladies, leave the purses behind. Sadly, pickpockets come to carnival also.
By the way, a lot of the "dirty" in New Orleans is created by visitors, not by locals. Yea, life is a little looser in the Big Easy, but jeez, some manners are in order, k? The Quarter has never been cleaner than in the last two years since the SDT company took over. Local heroes, they are. So come on down, pass a good time, treat the city as if it were your own home, and it'll love you right back!
